Terms and Conditions for Sarah Heaton Veterinary Physiotherapy

1. Services Provided :  Sarah Heaton Veterinary Physiotherapy offers specialized physiotherapy treatments for horses, including manual therapy, therapeutic exercises, laser therapy, kinesiology taping, and Winback Tecartherapy. All treatments are tailored to the individual needs of each horse.

2. Client Responsibilities

  • Veterinary Referral: A valid referral from a licensed veterinarian is required prior to initiating any physiotherapy treatment. It is the client's responsibility to ensure that their horse has been appropriately assessed by a vet.

  • Information Accuracy: Clients must provide accurate and complete information about their horse’s medical history and current condition. Failure to disclose relevant information may affect the effectiveness of treatment.

  • Appointment Attendance: Clients are responsible for attending scheduled appointments. If an appointment needs to be rescheduled or canceled, please provide at least 24 hours’ notice.

3. Fees and Payment

  • Charges: Fees for physiotherapy services are outlined in the service agreement or as communicated at the time of booking. Charges may vary based on the treatment type and duration.

  • Payment Terms: Payment is due at the time of service unless otherwise agreed upon. Acceptable forms of payment include cash, credit/debit card, or other methods specified by Sarah Heaton Veterinary Physiotherapy.

4. Liability and Health

  • Risk Acknowledgment: Clients acknowledge that while every effort is made to ensure the safety and well-being of the horse, physiotherapy treatments involve a degree of risk. Sarah Heaton Veterinary Physiotherapy shall not be held liable for any adverse reactions or injuries arising from the treatment.

  • Health and Safety: It is the client’s responsibility to inform Sarah Heaton Veterinary Physiotherapy of any changes in the horse’s health or condition. Treatment may be adjusted or discontinued based on the horse’s response or any health concerns.

5. Confidentiality All personal and medical information provided by clients will be kept confidential and used solely for the purpose of providing physiotherapy services. Information will not be disclosed to third parties without the client’s consent, except as required by law.

6. Treatment Effectiveness While physiotherapy aims to improve the horse’s health and performance, no guarantees are made regarding specific outcomes. Results may vary based on the individual horse’s condition, response to treatment, and adherence to recommended care plans.

7. Termination of Services Sarah Heaton Veterinary Physiotherapy reserves the right to terminate services if the client fails to comply with the terms and conditions or if the treatment is deemed no longer beneficial. In such cases, any outstanding fees will remain due.

8. Changes to Terms Sarah Heaton Veterinary Physiotherapy may update these terms and conditions as necessary. Clients will be notified of any significant changes, and continued use of services will constitute acceptance of the updated terms.

9. Governing Law These terms and conditions are governed by the laws of [Your Jurisdiction]. Any disputes arising under these terms will be resolved in accordance with the applicable laws.
